Sunny is licensed to provide Individual Community Living Supports (ICLS). ICLS is designed for eligible older adults who need support to keep living in their own home and community.
Start with eligibility and authorization
A case manager or lead agency explains eligibility, assessment and the authorized support areas. Sunny can then discuss current availability and how the service plan would be carried out.
Common ICLS support areas
- Household management and daily routines.
- Community access and participation.
- Health-related tasks within the authorized scope.
- Cueing, supervision and support that maintains independence at home.
