Do I need to know exactly which service I need?
No. You can start by describing the goal or support need. A case manager or lead agency helps determine eligibility and authorization, and Sunny can discuss whether the authorized service is currently available.
Does a 245D license mean services can begin immediately?
No. Availability, assessed need, authorization, required intake steps and the service plan still need to be confirmed.
Where does Sunny provide services?
Sunny is headquartered in Minneapolis. Contact us to ask about current availability in the community where support is needed.
Can family members be involved?
When appropriate and consistent with the person’s preferences and service plan, family members and chosen supports can be part of planning and communication.
What is the difference between IHS and IHS with training?
IHS focuses on assistance, cueing and support with familiar routines. IHS with training adds purposeful teaching and practice connected to skill-building goals in the support plan.
Can I email medical or other private records?
Please keep general email and website forms to non-sensitive inquiries. We will provide instructions for sharing private records through the appropriate intake process.
How do I make a referral?
Use the referral inquiry page or call 612-259-7524. A referral inquiry starts a conversation; it does not guarantee placement or service availability.
Is Sunny hiring?
We accept general employment inquiries year-round. Visit the Work for Us page to tell us about your interest and the area where you are looking for work.
